Fill out the form below. You will be contacted by a representative from West Michigan Cardiology within two business days to schedule your screening. Payment is due at time of screening and is non refundable. Basic screening package available at Grand Rapids and Greenville location. Coronary calcium score and Advance screening package only available at Grand Rapids location. Results will be mailed to the address provided within 14 business days of test completion.
All results are for screening purposes only. It is the patients responsibility to consult with their primary care provider if any abnormal results are detected. Preventative cardiovascular screening is not covered by insurance and there are no reimbursable options. No insurance claims will be filed. Physician and prior authorization are not required.
